For 30 years, Meditrax has done one thing without compromise: help older Australians take their medicines safely.

Meditrax is the first – and currently only – medication review service in Australia to integrate directly with My Health Record.

When its pharmacists complete a Residential Medication Management Review or a Home Medicines Review, their report uploads automatically to that patient’s My Health Record. Any authorised treating clinician can access it immediately – the GP, the specialist, the emergency department team – with full clinical context, not just a medication list.

The clinical work its pharmacists do for every resident and patient is now part of their permanent, accessible national health record. That is what connected medication management looks like in practice.
